POSITION: Service Coordinator

FLSA CLASSIFICATION: Non-Exempt

REPORTS TO: Director of Property Management & Facilities

POSITIONS SUPERVISED: N/A

POSITION OVERVIEW: This full-time position primarily assists elderly tenants of MHA's three senior properties by coordinating supportive services and is the single point of entry for aging services and other related services.

ESSENTIAL POSITION RESPONSIBILITIES:

1. Collaborate with housing management in meeting residents' needs; act as a liaison with families and caregivers; act as advocate for residents when appropriate.
2. Identify residents with unmet needs and facilitate access to available services and entitlement benefits when possible or develop new resources where feasible.
3. Coordinate services for residents in need of assistance with services such as health and wellness services, transportation, legal and financial, crisis control and promotion of recreational social activities.
4. Serve as an advocate for the residents if needed assisting with issues that may arise with Wichita Housing Authority, physician, family, etc.
5. Educate residents on application procedures, resident rights, and tenant obligations.
6. Work closely with the residents' association and individual residents to develop activities that meet social and health and wellness needs. Responsible for maintaining the monthly calendar for each property.
7. Recruit outside professionals to conduct educational programs and provide services such as health screenings, flu shots or podiatry visits.
8. Maintain an activity log and use the data to prepare regular reports on services provided monthly.
9. Help residents with open enrollment and enroll in the Kansas Low Income Energy Assistant Program.
10. Assist resident with annual recertifications for HUD housing application. This will include obtaining correct documentation and required signatures to complete the annual recertification.
11. Making monthly in-person home check-ins for residents at our 3 residential properties.

OTHER POSITION RESPONSIBILITIES:

1. Promote the mission and values of the Mental Health Association
2. Maintains an acceptable attendance record, to include department staff meetings, agency meetings, and training as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexibility in work schedule when needed.
3. Completing required timesheets, mileage sheets, expense sheets and other agency required paperwork accurately and on time
4. Exhibits an appropriate level of technical knowledge for the position.
5. Produces the quantity of work necessary to meet job requirements.
6. Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.
7. Maintain professional verbal and written interactions with peers, tenants, supervisors and other staff, which also includes effective listening skills. Use diplomacy and tact in dealing with difficult situations or people. Provide strong customer service skills, so that residents will have the best experience possible.
8. Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identify what needs to be done and takes appropriate action.
9. Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others.
10. Performs work conscientiously with a high degree of accuracy. Operate in compliance with personnel policies and practices.
11. Meets goals and objectives as mutually agreed upon during last performance review (if applicable).

POSITION REQUIREMENTS: The Service Coordinator is expected to have a bachelor's degree or appropriate social services work experience. Knowledge of and training in elderly and disability services with knowledge of referral processes for services. 2-3 years of social service delivery experience. Preferably working with elderly or people with disabilities. Being familiar with gaining, mental health and disability services to include local, state and federal entitlement programs.
