This is a professional staff position that provides direct services to older adults to promote diabetes education, and health and wellness education. The position will provide diabetes education, wellness education for adults 60+. The position is responsible for assessing the client's program eligibility for Diabetes Prevention Program (DPP) and Medicare Diabetes Prevention Program (MDPP) through the established program guidelines. The position will lead educational programming within the agency, partners and community members to promote 60+ health and wellness programming. This position will offer programming to promote health promotion and disease prevention, physical activity, a healthy lifestyle and positive aging.
