Responsibilities

As a Licensed Master Social Worker, you will:

• Provide essential social services to patients according to physician orders and established plans of care.
• Educate patients and their families, as well as collaborate effectively with the healthcare team.
• Deliver high-quality, compassionate care that aligns with our commitment to ethical practices.

Qualifications

Essential requirements:

• Master's degree from a Council on Social Work Education-accredited school.
• Current certification or license to practice in your state.
• Minimum of one year of social work experience in a healthcare environment.
• Basic technology skills, including mobile device operation.

Preferred qualifications:

• Experience in Medicare home health or hospice settings is a plus.

Additional requirements:

• Valid state driver license.
• Automobile liability insurance, compliant with laws.
• Dependable transportation in good working condition.
• Able to drive safely under various weather conditions.
• Current CPR certification.
