Diversicare - - Responsibilities: Assure the safety of each resident when transporting to and from the doctors office and/or hospital.; Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.; Sign in resident upon entrance and present residents documents to office personnel.; Obtain residents documents and any new prescriptions; check to ensure doctor has written on the order form; obtain follow-up appointment cards as necessary.; After returning to the facility, report to RCM and be prepared to answer questions concerning visit.
